We are looking forward to celebrating Maya and Mike’s wedding weekend together.

Dear Friends and Family,

To ensure the health and safety of all wedding guests, and so you can attend the Simcha, we require that you COVID-clear prior to our wedding weekend starting on December 17, 2021. 

We have hired a COVID-19 Health Safety company called VOW who will be handling the protocol as follows:

Everyone attending must provide BOTH of the following:

1. Vaccine Record

   1. Fully completed no less than 14 days prior to the date you are attending.

2. Negative COVID-19 PCR Test Results

   1. PCR Test from a Pharmacy or Testing/Healthcare Facility taken within 3 days of the first event you are attending. No at-home tests are accepted.

In the next 48 hours, on your smartphone:

1)   Please provide the most up-to-date cell phone numbers for you and everyone in your party that is attending to info@mayaandmike.com. 

2)    Have a photo of your Government-Issued ID and Vaccine Card ready to upload

3)    Click this {VOW Digital Health Event Link} and follow steps to COVID-clear.

**Should you need it, the Health Protocol ID is R8Q-KJ4

4)    Promptly upload PCR Test results when available.

*If you are attending Shabbat Dinner on December 17, 2021 you must take a PCR Test no earlier than Tuesday, December 14:

*If you are ONLY attending the Wedding on December 19, 2021 you must take a PCR Test no earlier than Thursday, December 16:

Note: Each guest must have their own account.  Kindly do not upload information for your spouse, plus-one, or colleague at any time.
